How old is the female and is she controllable?

You can't have a visiting female get pregnant.
A female sim that is already pregnant won't have the option.
Teen/YA can't try for baby without a back.
A female that is a couple of days shy of being an elder can't try for baby.

I installed the last patch yesterday, and tried for baby in two families. All went fine with all my hacks working.

Either a hack conflict, or one of the options Simmiecal mentioned.

There is a few naked hacks that require you get the sims in the mood before they can woohoo. First try to cuddle, then make out, and woohoo and try for baby should appear. If not, then force a error and see if that helps.

Inteen will let YAs get pregnant - not sure about JenFlowers Woohoo (both at http://www.insimenator.net/). You can always use something like Insim or Merola's mind control mirror to make a sim selectable. Just don't make a townie or NPC selectable and pregnant unless you want some problems (ok to do if you "move them in" after they get pregnant).
