Or does it only change the faces but keep the original genes?

It only changes the faces, but there is a really great tut somewhere on how to make plastic surgery genetic with Sim PE.  It is really easy and it works great.

I wonder why there's a sudden upsurge in people asking this question - I've posted this link four or five times this week in response to questions here and at other sims sites (mostly here).

Thanks for that link. Hopefully I manage to do it and not make my computer explode. I made all my sims have caricature long nose before and now I kind of regret it.
