I think it actually was a freak accident.
She was on a date, in a car, trying to make out with someone just as the date ended. I think the Make Out had been initiated when he suddenly stepped out of the car. No actual water baloon was ever involved, although I see that the woohoo memory immediately before was with a service sim, so that might very well have been the cow mascot. I can't remember; it was a long time ago.
